ApexRise is seeking county, Tribal and community-provider partnerships that help people maintain housing, strengthen everyday living skills and reduce the risk of returning to homelessness.

Service scope, staffing, coverage, capacity, funding and required approvals must be confirmed for each engagement before services begin.
Person-centered case management
Participant-defined goals, coordinated service plans, consent-based communication and regular progress reviews.
Housing retention
Tenancy-sustaining plans, lease responsibilities, early response to eviction risks and landlord coordination.
Independent-living skills
Practical support with household routines, budgeting, appointments, transportation planning and community connections.
Income and employment connections
Benefits navigation, job-search support and connections to training based on each participant’s goals.
Health and specialized-service coordination
Connections and follow-up with healthcare, mental health, substance-use treatment and other specialized providers. Coordination is not clinical treatment.
Language and cultural understanding
Support planning that respects language, culture, family and individual choice. Language staffing and interpretation arrangements are confirmed for each scope.

A useful proposal explains how support will be staffed, documented, funded and reviewed. These are the decisions to settle with the contracting partner before an agreement.

- Agree on the county, Tribal, prime-provider or Coordinated Entry referral pathway, eligibility responsibilities, intake steps and capacity checks. ApexRise does not independently enroll households into LTHSSF.
- Staffing, supervision and training
- The proposed model pairs a designated case-management contact with supervision and referral partners. Confirm caseloads, coverage, qualifying education or experience, background studies, and applicable program orientation and mandated-reporter training. Verify appropriate driver licensing if transportation is included.
- HMIS, privacy and documentation
- Confirm whether HMIS or another approved system applies, then agree on onboarding, authorized access, consent, data sharing, case-note standards and quality checks. HMIS access must be verified before any scope that requires it begins.
- Outcome reporting and review
- Agree on measure definitions, baseline information, reporting intervals and review responsibilities. Track participant goals alongside housing stability, referrals and income connections; set targets with the contracting partner.
- Procurement and fiscal controls
- Verify legal-entity records, W-9, insurance, applicable registrations and approvals. Agree on budget, authorized spending, billing documentation, invoice review, record retention and audit access before contracting.
