Case management, housing retention, independent-living skills and connections to community providers. Our proposed Permanent Supportive Housing model brings these services together around participant-defined goals; scope and availability are confirmed before services begin.

Start with the household's priorities.

Support often crosses more than one service. A housing concern can involve income, transportation, an unanswered application and an appointment with another provider. The first conversation is about the practical need and whether ApexRise has an appropriate role.

Tell us the city or county, the preferred language and the kind of help being sought. We discuss availability and the next step before requesting personal records. If several organizations are involved, we clarify their responsibilities so the proposed support fits alongside existing services.

A clear plan before services begin

A service agreement should explain the work, how often contact will happen, who is responsible for each action and how progress will be reviewed. The participant's own goals should be part of that plan. Consent and communication arrangements should be discussed before coordinating with other providers.

Funding also needs to be clear. Ask who will pay, whether there is a cost to the household and whether an agency must authorize the work. Do not assume that a public benefit covers a service because the names sound similar.

Choose the right kind of help

These services focus on planning, navigation and coordination. They do not replace emergency services, legal representation or clinical care. If a notice or appointment has a deadline, say so when seeking help and contact the agency or qualified professional responsible for that issue.

Households can ask about support directly. Professionals can start a referral conversation without sending sensitive case records. Both routes begin with confirming the fit, not a promise of placement or enrollment.

Who pays for a service?
Funding depends on the specific arrangement. Confirm who is responsible for payment, any household cost and any required authorization before services begin.
Can I enroll in a county housing program through this website?
No. A website inquiry does not decide public-program eligibility or enrollment. The responsible agency may require its own assessment, referral and capacity checks.
Does ApexRise offer mental-health or substance-use treatment?
No clinical treatment is offered through these services. Support means referral and coordination with appropriate healthcare, behavioral-health and community providers.
