For Permanent Supportive Housing partnerships
Our proposed service model focuses on maintaining housing and reducing the risk of returning to homelessness. A county, Tribal or prime-provider agreement would define the population, referral pathway, staffing, capacity, documentation and outcome measures. ApexRise does not represent this model as an awarded or currently enrolled public program.
Who this support is for
Some households need help understanding a housing search or application. Others have a home but face a problem that could put it at risk. Those are different needs, and the first conversation should identify which one you are facing.
Tell us the county or city, the general concern and whether a notice or appointment has a deadline. You do not need to email a lease, medical record or detailed case history to ask about availability.
What tenancy support looks like in practice
A housing-retention plan can address lease responsibilities, household routines, budgeting and communication with a landlord. With the participant's consent, coordination can help clarify a concern, identify the right contact and keep track of agreed next steps.
If a repair, unpaid balance or notice raises a legal question, the next step is advice from an appropriate legal service. ApexRise can help identify the route and prepare questions; we do not decide a tenant's legal position.
The aim is to respond early to housing concerns and support participant-defined stability. We cannot guarantee that a landlord will agree to a request or that a funding program will approve an application.
Our service and the state's Housing Support benefit
This page describes ApexRise's service scope. Minnesota's Housing Support program, formerly GRH, is a separate benefit for eligible people in approved settings. ApexRise is not an enrolled Housing Support provider.
Our proposed Permanent Supportive Housing model is a basis for partnership discussions. A specific agreement would establish who is served, how referrals arrive, staffing, capacity, documentation and funding. A website inquiry does not enroll someone in that model.
Preparing for a housing conversation
Bring questions about the next decision: which application is appropriate, whether a setting is approved, what a landlord is asking for or which organization can help with a deadline. Ask the receiving agency what documentation it needs before sending personal records.
Housing search, financial assistance and supportive services may involve different organizations. Clarifying that division of work can prevent an unanswered rent question from being mistaken for an approved housing plan.
Availability, funding and costs
Before services begin, confirm the scope, who will provide it, the funding arrangement and any cost to the household. Contacting ApexRise does not confirm eligibility or enrollment in a publicly funded program. The responsible agency may require an eligibility decision, an authorized referral and confirmation of capacity.

- Can ApexRise pay my rent?
- ApexRise does not provide rental-assistance funds directly. We can discuss navigation to agencies that assess eligibility for financial help.
- Is this Minnesota's Housing Support program?
- No. This page describes a service scope. The state Housing Support benefit has separate eligibility, provider and setting requirements, and ApexRise is not enrolled as a provider.
- Can you guarantee a home or prevent an eviction?
- No. Housing availability, applications, legal decisions and financial assistance are controlled by other parties. We discuss practical support without promising an outcome.
- What should I do with a landlord notice?
- Keep the notice and get advice promptly from a tenant legal service such as HOME Line. Ask about any deadline on the document. ApexRise does not provide legal advice.
