How this differs from a referral list

A list is a phone number. Navigation means we help you understand what the program does, whether you are likely to qualify, what documents to gather, what the application asks, and what happens after you submit it. Where it makes sense, we follow up to confirm the connection was made.

Who navigation can help

Navigation can help when you have been given several phone numbers but still do not know which agency handles your question. It also helps when an application is waiting for information and you are unsure what the request means.

You do not need to know a program's name before asking a question. Start with the practical need, such as keeping housing, arranging transport or understanding a benefits letter. We discuss whether navigation is available and appropriate for that situation.

Working through an application

First, identify the responsible program and what help it actually offers. Then ask about the application route, required information and any time limit. The agency decides eligibility; our role is to help organize the questions and next steps.

Document preparation means checking what is required, how to submit it safely and what to ask when something is missing. We do not assume that a document accepted by one program will satisfy another.

After the referral

A useful referral includes a next action and a way to follow up. Depending on the agreed service, that can mean checking whether the receiving agency has the inquiry, helping prepare for a call or identifying who can explain a decision.

If an agency cannot help, ask why and whether a different route may fit. A closed program, lack of capacity and an eligibility decision are different situations. Understanding the reason helps determine the next question.

Working across services

Housing, income, food, transportation and health appointments often affect one another. Navigation can help prioritize what needs attention first and connect the household with the appropriate provider for each need.

For mental-health, substance-use or healthcare concerns, support means connection and coordination with qualified providers. For legal or immigration questions, it means identifying legal assistance. It does not mean that ApexRise assesses, treats or represents the person.

Availability, funding and costs

Before services begin, confirm the scope, who will provide it, the funding arrangement and any cost to the household. Contacting ApexRise does not confirm eligibility or enrollment in a publicly funded program. The responsible agency may require an eligibility decision, an authorized referral and confirmation of capacity.

Should I send all my documents with an inquiry?
No. Do not send sensitive records through general email. We first discuss service fit, consent and an appropriate way to share any information that is needed.
